•  Our ACO uses the SNF 3-Day Rule Waiver, pursuant to 42 CFR § 425.612.

Parties to the arrangements: The ACO has entered into agreements with the following parties (hereafter referred to as the "Providers"):

• Ideal Home Health Services, LLC; and
• Odyssey HealthCare of Flint, LLC (d/b/a Gentiva), Odyssey HealthCare Operating B, LP (d/b/a Gentiva I), and SouthernCare, Inc. (d/b/a Gentiva Il).


Date of the arrangements: February 1, 2024


The type of item, service, good, or facility provided under the arrangements: The Providers will make payments to the ACO that will be used to defray costs incurred by the ACO to acquire and/ or develop care management software, data access and analytic tools, and other infrastructure investments and marketing expenses that support the delivery of quality care to the ACO's assigned beneficiaries.


The ACO's Board of Managers has duly authorized the arrangements and made a bona fide determination that the arrangements are reasonably related to the purposes of the Medicare Shared Savings Program because they will promote investment in infrastructure and redesigned care processes for high quality and efficient service delivery for ACO beneficiarios.
