Use the form to tell us about an independent primary care practice or health center that you have a relationship with and earn up to $1,000.
As a friendly reminder, an eligible practice or health center is someone who:
- You have an existing business or personal relationship with and agree to provide some form of personal support for the connection
- Is an independent, primary care practice or health center with no hospital or health system ownership
- Uses Meaningful Use-certified EHRs and accepts Medicare
- Has the ability to participate with Aledade in one or more Medicare or Commercial products (patient minimums may apply)
