In first two years, practices improved care for over 41,000 patients, resulting in nearly $14 million in shared savings.
JACKSONVILLE, Fla., and BETHESDA, Md. — September 7, 2023 — Florida Blue, the state’s Blue Cross Blue Shield plan, and Aledade, the largest network of independent primary care providers in the country, today announced that their collaboration on a value-based care network in Florida helped improve the quality of care and patient outcomes for tens of thousands of commercially-insured members over two years of partnership. Led by Florida physicians, the collaboration resulted in nearly $14 million in shared savings earned by this network of nearly 60 primary care practices, federally-qualified health centers, rural health clinics and multi-specialty clinics.
Value-based care networks, or Accountable Care Organizations (ACOs), are groups of providers that share financial responsibility for delivering high-quality, better-coordinated patient care. Results from the Aledade-Florida Blue ACO show the 59 primary care practices that participated in 2022 outperformed the market in rates of control of diabetes, as well as screening rates for breast cancer, colorectal cancer and cervical cancer. The practices also increased their rates of patient outreach through workflow management powered by the Aledade App, connecting with 3 out of every 4 patients following an inpatient visit. This follow-up is a critical way to enhance the relationship between these practices and the more than 41,000 patients served under this collaboration.
Additionally, between year one and year two, these practices reduced hospitalizations by twice as much as other practices in their region (a 15 percent reduction compared to 7 percent) and visits to the Emergency Department (a 1.8 percent reduction compared to 0.8 percent).
