In 2016 the Kentucky ACO started with 8 practices in Western KY. Our ACO now has 22 practices from the far west to central KY and 1 in Il. They take pride in quality care while leading in the Core 4.
Our practices are pillars in the community and have led initiatives and events. We collaborate with hospitals, SNF/rehab & health departments for quality, consistent care focusing on wellness.
