This June, join us for our second annual Tennessee and Virginia CHC summit! During this two-day event, attendees will have the opportunity to network, engage in educational sessions, earn Continuing Medical Education (CME) credits and win prizes!

Sessions at this year’s summit will focus on the strategies, tools and actions you can use to improve population health in the value-based care model.

Note: Special interest sessions begin at 11 a.m. ET on Thursday, June 5 and the summit ends at 3 p.m. ET on Friday, June 6. Please book your hotel room accordingly if you plan to travel either the day of or the night before the summit begins.

• The Hotel Madison and Shenandoah Valley Conference Center is located at 710 S Main St. Ste 149, Harrisonburg, VA 22801

• Summit sessions will take place on the first and second floor of the hotel in the conference spaces. Signage and Aledade staff will be there to help guide you

• Check-in time at Hotel Madison is after 4 p.m. and check out time is before 11 a.m.

Friday Luggage Service

• On Friday, hotel check out is 11 a.m.

• If needed, you may check your luggage is room Appalachia B, across from our main ballroom, and pick it up before you leave

The summit will include both administrative and clinical tracks. Keynote, networking, and awards sessions will also be included.

This will be a fully in-person event with no virtual attendance option.

• Registration opens 9 a.m. Thursday

• Special Interest Sessions start at 11 a.m. Thursday, followed by lunch at noon, and the Welcome Session at 1 p.m.

• After the final session at 5:30 p.m. Thursday, we'll have a reception with foods, drinks, a DJ and karaoke until 8 p.m.

• Sessions ends at 3 p.m. on Friday. Make sure to stay til the end for final awards and raffle prize drawings
