Keystone Rural Health Consortia, a community health center (CHC) in PA, saw a measurable positive impact by focusing on data-driven preventive services. By using the Aledade App to identify individuals in need of Medicare Annual Wellness Visits (AWVs), transitional care management (TCM), and other preventive care, they were able to ensure that these individuals had access to all the resources they need to make more engaged, informed decisions about their health.
